Myeloid Sarcoma of the Bladder in a Patient with Chronic myelomonocytic Leukaemia

BackgroundMyeloid sarcoma is a rare extramedullary tumour of immature granulocytes, most commonly involving the skin, bone, lymph nodes, and soft tissue. It usually associated with diagnosis relapsed or de novo acute myeloid leukaemia, lymphoblastic transformation myelodysplastic/myeloproliferative neoplasm, can occur as isolated sarcoma.Case reportA 66-year-old female 7-year history stable chronic myelomonocytic leukaemia presents urgency, frequency, dysuria symptoms, without new constitutional symptoms. She found to have atypical, multifocal lesions on right posterolateral wall bladder hydronephrosis. Pathology reveals sarcoma; surprisingly, bone marrow evaluation does not show evidence leukaemic transformation.ConclusionsMyeloid occurring in patients extremely rare, there are no cases reported English literature these developing bladder. The urological manifestations an underlying haematological malignancy best managed combination systemic chemotherapy allogeneic stem cell transplant, this case, only surgical intervention required was ureteric stenting tissue biopsy. Although it essential consider alternative diagnoses when confronted atypical tumour; failure do so may result patient harm by exposure unnecessary delay potentially curative treatment.
